Prof. Amadei already approved my questionnaire and asked me to also gather the data in other areas such as water, food, transportation, and communication because that also related in energy sector. Prof. Amadei asked me to read his book especially on chapter: community appraisal and capacity.

The questionnaire was a template of key performance indoicator (KPI) for rural electrification. I add some information so it will be more complete in collecting data.
